METHOD FOR REMOVING THE DEFECT OF TRACHEAL AND LARYNGEAL WALL Russian patent published in 2007 - IPC A61B17/24 A61F2/04 

FIELD: medicine, reconstructive microsurgery, thoracic surgery.

SUBSTANCE: at first, it is necessary to sample cartilaginous fragments and fulfill wedge resections in their middle part by keeping one of the surfaces. After that, due to applying sutures one should bring together cartilaginous edges in area of resection by forming arch-shaped transplants. Then, in donor area at fascial surface in projection of supplying vessels of sampled autotransplant one should form the beds as separate canals for pre-obtained cartilaginous fragments at keeping septa between them with perforating vessels. Into the beds developed one should put transplants with their convex kept surface towards fascia, and their concave one - towards skin with subsequent fixation of cartilaginous fragments with sutures to autotransplant's skin. The developed autotransplant should be sampled as a single complex together with cartilaginous fragments to be placed into the defect its skin being inside at the support of rigid component onto anterior and lateral walls of the defect. Circulation in autotransplant should be reconstructed and cartilaginous fragments should be fixed with sutures to tracheal and/or laryngeal cartilages. The innovation enables to reconstruct anatomical tissue ratio and physiological tracheal lumen, prevent tissue shift in early post-surgical period and formation of bedsores at opening a cartilage.
