SUBSTANCE: the present innovation deals with treating gunshot defects of soft tissues and a femur. At the first stage in iliac area it is necessary to isolate the fragment of iliac crest - unvascularized osseous transplant - of required volume depending upon the defect's size. Then this fragment should be transferred under thoracodorsal muscle at any parts of its distribution, for forming a cutaneous-muscular-osseous transplant. On healing iliac wounds and those in the area of forming cutaneous-muscular-osseous transplant it is important to start the second stage of the operation in question. One should take a vascularized thoracodorsal cutaneous-muscular-osseous transplant with the ingrown osseous fragment of iliac crest to transfer it into the area of femoral defect that increases the efficiency in treating patients with extensive gunshot defects of soft tissues and femur complicated with suppuration of femoral wound and enables to decrease traumatism and the number of stages during operation.

EFFECT: shortened terms of therapy and rehabilitation.
