SUBSTANCE: the present innovation deals with surgical treatment in patients with complicated extra-sphincter pararectal fistulas associated with insufficiency of anal sphincter. It is necessary to dissect fistular tract together with surrounding cicatricial tissues as a single unit with cicatricially altered parts of external anal sphincter and puborectal muscle together with perianal skin and adjacent anal wall. In accordance with the dislocation and the size of the developed sphincter defect it is important to mobilize a full-layer mucous-muscular fragment of rectal wall, mobilize and suture together unaltered parts of external sphincter by reconstructing anal sphincter, and unaltered parts of puborectal muscle by reconstructing it, also. The wound of perianal skin should be sutured in radial direction followed by descending the mobilized fragment of rectal wall to fix it towards the edge of perianal skin. The innovation enables to remove complicated extra-sphincter pararectal fistula and available anal insufficiency in the course of one operation, exclude the contact of the reconstructed sphincter and puborectal muscle with fecal masses.
