FIELD: surgery and coloproctology.
SUBSTANCE: semilunar incision is made along the anocutaneous fold 5–6 cm long in the projection of the internal fistula opening. The muco-muscular flap is mobilized for 5–6 cm. The distal part of the fistulous tract is isolated and stitched with a thread-holder, through which a sharpened trocar port with a diameter of 1.0 cm is inserted into the wound. Pulling the thread-holder, advance the port along the fistulous tract in a circular motion, highlighting the external fistulous tract to the subcutaneous portion of the external sphincter. The external fistulous tract is tied up and cut off, then the mobilized flap is brought down to the level of the anocutaneous fold and fixed to the perianal skin.
EFFECT: method allows to improve the results of treatment of patients with transsphincteric fistulas of the rectum, reduce the intensity of pain in the postoperative period, shorten the time of wound healing in the perianal region, reduce the length of stay of patients in the hospital and contribute to faster recovery due to economical excision of the external fistula using a trocar port.
