FIELD: medicine.
SUBSTANCE: invention concerns medicine, namely surgery. Access to organs of abdominal cavity of the patient, retroperitoneal space and large vessels at the combined surgical intervention, including cut of skin with subject tissues is made. Thus the cut begins from the point located lower 2 cm of an apex of xiphoid process on median line, the cut is continued in parallel right costal arch 2 cm below edge of ribs to the point located 2 cm more lateral than the right midclavicular line, conditional piece is defined on the right anterior auxiliary line in such a manner that its top border is the bottom edge of X right rib, and bottom - forward arista of ileal bone, length of this conditional piece is measured, dividing it into 3 equal parts and continuing the cut to the conditional point being the top third of the specified piece, the second conditional piece is defined on the right back auxiliary line, and the bottom edge of X right rib is its upper border, and bottom - conditional point of crossing of the right back auxiliary line with wing of the right ileal bone; the piece is measured, its middle is defined and the cut is continued to median point of the second conditional piece, Thus, slanting cut with crossing of epigastric artery and vein is formed on the right, hemostasis is thus reached by their ligations, after a section of muscles and preperitoneal fatty tissue, parietal peritoneum is dissected, separated from subject muscles and displaced in the medial party in such a manner that the section piece is on the right back auxiliary line with the top border - crossing of bottom edge 6 segments of the liver with a back auxiliary line, and bottom-crossing of the specified line with a wing of ileal bone; in case of perpendicular locating of subcostal and iliolumbar nerves of a line of the specified cut, their crossing carry out with preliminary alcoholisation of nervous trunk and the subsequent realisation of hemostasis in perineural tissues.
EFFECT: method is more convenient for the patient, less traumatism, allows to execute operation both on organs of abdominal cavity, and on organs of retroperitoneal space, is technically simple in execution and does not demand hi-tech and expensive equipment.
