FIELD: medicine.
SUBSTANCE: invention relates to surgery and can be applied for access for operations on liver and biliary tract. Skin cut is performed from linea parasternalis on the left 5 cm lower than rib arch and is continued on arch directed upward and rightward through the line located on the border in upper and middle third of abdomen line between xiphoid process and navel to right linea parasternalis, further cut is directed towards cartilage part of X rib, forming arch, directed downward and is continued along upper edge of X rib to anterior axillary line. Aponeurosis is exposed and transected in the same direction. Right rectus is exposed, lifted and cut in transverse direction closer to tendinous intersection of the second segment. In lateral part of wound successively cut is external oblique muscle of abdomen, internal oblique muscle of abdomen and transverse muscle of abdomen, transverse fascia and peritoneum. In medial part of wound intersected is white line of abdomen, transverse fascia and peritoneum to medial edge of left rectus. Tomy of rib arch in region of X rib is performed. Intercostal muscles are intersected in IX intercostal space to anterior axillary line, realising access.
EFFECT: method makes it possible to reduce trauma, reduce term of wound healing.
