FIELD: medicine; surgery.

SUBSTANCE: eventration therapy involves intra-abdominal arrangement of a transplant in a septic wound to be fixed to anterior abdominal wall. The area of mesh endoprosthesis is evaluated individually at excess wound area by 5-7 cm with threshold intra-abdominal pressure 12-13 mm WC and less. Endoprosthesis is fixed with polypropylene nonabsorbable monothread by an endoscopic glover's needle delivered through all the layers of anterior abdominal wall with subcutaneous tying and immersing the knots. The second suturing is distanced 2-2.5 cm from the prosthesis edge with covering internal leaf of rectus aponeurosis. A wound is filled with bulky ointment tissues without coetaneous suturing. The postoperative period involves wound management with the open method. The defect is closed completely as far as the wound is cleansed, healthy granulations are observed on the endoprosthesis, and the intra-abdominal pressure is decreased to 5-7 mm WC.

EFFECT: reduced percentage of postoperative complications by ensured frame function of abdominal wall, prevented abdominal compression syndrome, possibility for dynamic supervision and adequate surgical sanitation of the septic wound and more active postoperative patient management, prevented repeated operations of postoperative ventral hernias.
