TRANSOSSEOUS APPROACH TO COTYLOID CAVITY Russian patent published in 2010 - IPC A61B17/56

Abstract RU 2392891 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, particularly to traumatology and orthopaedics. An incision along an iliac crest towards the anterosuperior spine, passing onto the anteroexternal surface of the thigh, extending to the posterosuperior spine of the iliac bone is followed with a vertical osteotomy of the anterosuperior spine of the iliac bone. A cut off fragment with the tailor's muscle and inguinal ligament attached is moved in a medial direction. The osteotomy of the anteroinferior spine of the iliac bone is performed at the attachment of a tendon of the rectus muscle of thigh. The cut off fragment is mobilised in a medial direction. A neurovascular bundle is separated and mobilised. An internal surface of a roof of the cotyloid cavity is cleared by skeletonisation of the abdominal oblique muscle and the iliac muscle. The capsulotomy follows. Under direct vision, reduction in hip dislocation, reposition of the roof of the cotyloid cavity and fixation are performed. The capsule of the hip joint is closed. Then a compression osteosynthesis of the anteroinferior spine of the iliac bone with the rectus muscle of thigh attached is performed with mounting the first S-back compression clip with short and long protective supports made of a thermal shape memory metal. The compression osteosynthesis of the anterosuperior spine of the iliac bone with the tailor's muscle attached is ensured with the second S-back compression clip. A wound is closed layer-by-layer.

EFFECT: method allows keeping integrity of the tailor's and rectus muscles, ensuring good vision of the performed manipulations, conditioning the complete inspection of the hip joint, and reduces risk of neurodystrophy of the hip joint.
