FIELD: medicine.
SUBSTANCE: invention refers to medicine, particularly to urology and can be used for surgical treatment of phimosis or premature ejaculation. Beforehand, penis is irrigated all around with an anaesthetic solution easily penetrating deep through the intact skin. After the beginning of anaesthesia, skin of penis, pubis, scrotum and internal surface of thighs is washed with 4% aqueous sodium hydrogen carbonate and 3% hydrogen peroxide at temperature from +37 to +42°C with using sterile gauze tampons. A cleaning agent is removed, and the skin is dried up with sterile air and wiped with the sterile gauze tampons plentifully moistened with warm 0.5% aqueous chlorhexidine. A sterile tourniquet is applied. On the dorsum of penis, there are 2 areas distanced 3-6 mm to the right and to the left of a midline and distanced 6-10 mm from the tourniquet to make a series of skin punctures with an injection needle at an angle 15-30° towards the base and in a plane parallel to the midline and to introduce the anaesthetic solution cooled to +18 - +20°C for nerve block in the subcutaneous fat that is controlled for infiltration by a temperature control unit up to complete fusion of two medication infiltrates on the dorsum of penis. A skin incision starts in a lateral surface of the penis. Beforehand, the skin of specified area along the section line is folded with fingers of the left as a triangle with its base downwards. The fingers of the right hand hold a scalpel with its cutting edge up to be immediately introduced to the triangle base between the fingers of the left hand perpendicularly to the axis of penis tangentially. Under the skin lifted with the cutting edge, the scalpel performs periodic tangential reciprocations relative to a surface of the cavernous body when cutting the skin of external leaf of foreskin and balanus when cutting the skin of internal leaf of foreskin perpendicularly to the axis of penis directly under the target section line, at first pushing along the full length of the cutting edge, then pulling out for 2/3-3/4 of its length and cutting the skin up to an end of a circular incision of external and internal leaves of foreskin. After suturing, the tourniquet is removed, and a warm coagulating haemostatic preparation with antiseptic activity is rubbed into the suture region with using a soft brush until complete haemostasis.
EFFECT: invention improves liquefaction and removal of thick pus, smegma and skin secretion, provides higher accuracy, efficiency and safety of anaesthesia, skin incision and prevents suture haemorrhage.
