FIELD: medicine.
SUBSTANCE: invention relates to field of medicine, namely to endoscopic rhinosurgery, and can be applied in cases of stopping bleeding from posterior parts of nasal cavity. Essince of method lies in exposure of sphenoid-palatine artery and its branches under endoscopic control, with their further selective cauterisation. Separation of mucoperiosteal flap is started more anteriorly than lower part of vertical plate of medial nasal turbinate, more posteriorly than posterior fontanella of maxillary sinus and is performed until perpendicular plate of palatine bone and ethmoidal crest are exposed. After that cauterisation of sphenoid-palatine artery with bipolar coagulant is performed. After that, mucoperiosteal flap is separated downward from sphenoid-palatine foramen and backward to the level of upper edge of choana. If foramen accessorium with ascending posterior septal artery is detected, its cauterisation is performed. Then, mucoperiosteal flap is separated above the level of sphenoid-palatine foramen backward to anterior wall of sphenoid sinus in superior nasal meatus. Cauterisation of branches of sphenoid-palatine artery in superior nasal meatus is performed. After that, mucoperiosteal flap is laid into place. It is possible to fix mucoperiosteal flap with fibrin glue.
EFFECT: application of claimed invention makes it possible to increase efficiency of selective cauterisation of sphenoid-palatine artery in case of bleeding from posterior parts of nasal cavity.
