SUBSTANCE: invention refers to medicine, namely to neurosurgery, and can be used for amygdalohippocampal complex removal in tumours of pole and medial parts of temporal lobe or sclerosis of hippocampus. Approach to medial parts of the middle cranial fossa is performed. Access to the hippocampus is ensured by resection of the temporal lobe through a pterygopalatine fossa and a wedge-like sinus by resection of the base of the pterygoid process and a large wing of the wedge-like bone.

EFFECT: method enables safe amygdalohippocampectomy perform, enables avoiding cosmetic defects associated with temporal muscle injury, frontal branch of facial nerve due to use of minimally invasive and low-traumatic lateral extended transsphenoidal endoscopic access through pterygomaxillary fossa.
