METHOD FOR FIXING VAGINAL VAULT TO UTEROSACRAL LIGAMENTS IN SURGICAL MANAGEMENT OF GENITAL PROLAPSE FOLLOWING VAGINAL HYSTERECTOMY Russian patent published in 2014 - IPC A61B17/42 

FIELD: medicine.

SUBSTANCE: formed vaginal vault is fixed to uterosacral ligaments after a vaginal hysterectomy. A bilateral suture is applied. Pubocervical and rectovaginal fascias are combined. One suture is extended to underrun a vesicouterine fold of the peritoneum, the pubocervical fascia and vagina. The tissues are fixed: within '1 o'clock' of an anterior fornix of the vagina; within '2 o'clock' of a vaginal wall; a left uterosacral ligament in the caudal direction from an ischial spine; within '5 o'clock' of a Douglas pouch of the peritoneum and a retrovaginal septum with a needle pierced out into the vagina. Similarly, an opposite end of the suture is delivered symmetrically through '11 o'clock', '10 o'clock', a right uterosacral ligament and '7 o'clock'. The suture ends within a posterior fornix of the vagina are tightly knotted.

EFFECT: method enables using one suture to form the vaginal vault and close an abdominal cavity, reduces a risk of tissue ischemia and wound infection, optimises a technique of apical vaginal repair; applying the Douglas pouch with the uterosacral ligament preventing developing enterocele.
