FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to surgical gynaecology. A vaginal hysterectomy involves dissection of a right ischiorectal fossa. Under the visual control at 1.5 and 2.0 cm from the sciatic spine, the sacrospinal ligament is underrun twice with a ligature the ends of which are fixed to the stumps of the cardinal-sacral-uterine complex on the left and on the right that is followed by peritonisation and formation of the vaginal vault. Further, after the hydrodissection and cutting out the rhomboid flap in the perineum region, the perineal tendon centre and the dorsal edges of the bulbospongiosus perineum muscles are mobilized, posterior wall of the vagina is mobilized from the rectovaginal fascia, in the formed submucosal canal, previously fixed to the sacrospinous ligaments and the vaginal vault, the ligatures are brought out into the perineum, after partial mucosal closure within a colpotomy wound, the above ligatures are tightened in tension with restoring a physiological position of a vaginal vault; the operation is completed with colporrhaphy and colpoperineolevatoroplasty.

EFFECT: method allows to make single-step correction of apical prolapse of pelvic organs during vaginal hysterectomy, to prevent its recurrence, minimize intraoperative complications due to possibility to perform manipulations under control of vision, to avoid implant-associated complications in postoperative period and to reduce cost of intervention due to rejection of use of polypropylene endoprosthesis.
