FIELD: medicine.
SUBSTANCE: pleurotomy is performed in a patient lying on his/her back with bolsters placed under the patient; one of the bolsters is placed under the shoulders, and another one is placed from the approach side, under the lateral side in parallel with the spinal axis. The pleurotomy is performed after incising the skin, the subcutaneous fat and fascia along the second intercostal space from the lateral border of the sternum to the anterior border of the deltoid muscle, dissecting the intercostal muscles in the second intercostal space, transecting the second or third rib one of which is specified guided by the nearest proximity of one of the ribs to a projection of the root of the lung on a surface of the sternum. Thereafter, the wound edges are brought apart, and tissues found in the operative filed in a projection of the pulmonary artery and the pleura are dissected; the artery with proximal segmental branches is separated.
EFFECT: method enables reducing operative injuries, improving visualisation of the manipulations by reducing the wound depth, increasing an approach angle to the proximal segmental arteries, increasing a free space and ease of operation for an operating surgeon and an assistant.
