FIELD: medicine.
SUBSTANCE: investing tissues are dissected 2-3 cm below the costal arch border in the pararectal direction downwards stopping 5-6 cm short of a horizontal line of the navel. The approach extends in the lateral direction, and the investing tissue incision continues to a cross point of an anterior axillary furrow and the costal arch border. A layer opening of the anterior abdominal wall is combined with dissecting the rectus sheath at its lateral edge along the full length of the pararectal portion of the incision. The rectus muscle is mobilised and brought in the medial direction; the iliohypogastric nerve is mobilised and brought in the lateral direction. The wound edges are opened to approach an operated organ.
EFFECT: method provides reducing a number of postoperative complications, such as disturbed innervation of the muscles of the anterolateral abdominal wall, postoperative herniation, prolonged pain syndrome ensured by the optimum laparotomic approach to the upper abdominal organs, wherein the rectus muscle and the iliohypogastric nerve are mobilised and retracted.
