SUBSTANCE: identification document in the form of a booklet comprises a plurality of sheets, which are folded or bent substantially along corresponding longitudinal lines to obtain booklet pages. The identification document comprises: a cover sheet; electronic data storage means embedded in the cover sheet and equipped with a radio transmission antenna, and a data sheet, different from the cover sheet and formed by first and second plastic layers, wherein the data sheet is situated within the cover sheet and forms a data page containing identification data of the subject, and a connecting page attached to the cover sheet. The data sheet further includes an intermediate layer of flexible material, situated between the first and second layers of plastic and passing between the first and second layers at least along the longitudinal fold line. Said sheets are connected to each other in corresponding fold lines. The electronic data storage means is situated at the opposite side of the data page relative to the fold line.
