FIELD: medicine.

SUBSTANCE: all muscular fibres of inferior oblique muscle are dissected from sclera without preliminary sewing, then inferior rectus muscle is fixed and, with orientation on its external edge, stump of inferior oblique muscle is sewn to sclera with interrupted suture. The degree of anterior transposition of inferior oblique muscle depends on the value of angle of vertical deviation: in cases when the value of vertical angle does not exceed 7° by Hirschberg, inferior oblique muscle is sewn to sclera 2 mm lower than the level of fixation of inferior rectus muscle and 1 mm outwards from its lateral edge; if angle of vertical deviation is more than 7° by Hirschberg, inferior oblique muscle is sewn to sclera at the same level as place of fixation of inferior rectus muscle and 1 mm outwards from its lateral edge.

EFFECT: reduction of treatment trauma, reduction of manipulation duration, facilitation of operation performance due to refusal from performing complex manipulations near posterior eye pole, simplification of method for calculation of the degree of anterior transposition of inferior oblique muscle depending on the value of vertical deviation in adduction of eye ball, increased accuracy of vertical deviation correction due to weakening of inferior oblique muscle in case of large and small angles of deviations in different volumes, improvement of functional outcome of operation.
