FIELD: medicine.
SUBSTANCE: lateral thoracotomy is performed in the 4 or 5 intercostal space. The skin, subcutaneous fat, intercostal muscles are cut by layers. The lung is selected down to the root. The pulmonary artery, the upper and lower pulmonary veins are double-ligated and crossed. The main bronchus in its distal part is sutured. Pneumonectomy is performed. After lung removal, bronchial stump reamputation is performed at the level of the mouth. The lateral tracheal wall is sutured manually using an atraumatic thread. Bronchial stump suture tightness is checked with the lung inflated under high pressure. Size and boundaries of pulmonary hernia are assessed. Mediastinal plastic is performed at the reducedrespiratoty volume. Z-shaped sustures are made in the mediastinal pleura between the pericardium and the chest wall to eliminate hernia sac. The pericardium is fixed by U-shaped sutures to the chest wall along the parasternal line.
EFFECT: prevention of postpneumectomic syndrome, formation of mediastinal pulmonary hernia due to hernial ring elimination.
