SUBSTANCE: invention relates to medicine, namely to thoracic phthisiosurgery. A lateral thoracotomy is performed in the seventh intercostal space. The skin, subcutaneous fat, intercostal muscles are dissected in layers. The parietal pleura is exfoliated in the extrapleural layer with the formation of sheets with dimensions comparable to the hernial orifice. The lung and abdominal organs are isolated from adhesions. The size and length of the hernial orifice are assessed. The abdominal organs located in the chest cavity are adjusted. The remains of the resected diaphragm are isolated. After that, the free edges of the parietal pleura are sutured to the remnants of the edges of the resected diaphragm with separate U-shaped nylon sutures. The restored dome of the diaphragm is strengthened with a polypropylene mesh implant, fixing it to the seam and areas of the surface of the restored diaphragm with separate prolene sutures.

EFFECT: method makes it possible to effectively replace extensive diaphragm defects after its subtotal removal during pleurectomy for tuberculous empyema of the diaphragmatic pleura, to ensure mechanical reliability and biomechanical extensibility of the implant and adequate ventilation function.
