FIELD: medicine.
SUBSTANCE: pre compression-distraction osteogenesis is performed by cutting through the transitional fold of the oral mucosa from the retromolar area to the second premolar of the lower jaw. Then the lower jaw branch and/or body is skeletonized. The outer cortical layer is cut, and the outer and inner cortical plates on the front and rear edge of the lower jaw branch are cut. An intraoral extramedullary compression-distraction device is installed which is fixed with mini-screws passing through the two cortical layers, followed by inner cortical bone fracturing, bone fragments compression and lower jaw distraction. After the retention period, bilateral sagittal-plane osteotomy of the mandible is performed with removal of compression-distraction devices and upper jaw osteotomy with its subsequent extension forward with further upper and lower jaw fixation in orthognathic position. Then, by lower jaw trapezoidal osteotomy with chin forward projection, augmentation genioplasty is performed.
EFFECT: method allows to increase the missing bone tissue amount in the lower jaw body and branch areas, as well as to reduce process trauma due to gradual stretching of the inferior alveolar nerve in order to create conditions for further orthognathic surgery.
