FIELD: medicine.
SUBSTANCE: invention refers to medicine, particularly to dentistry and maxillofacial surgery. After anaesthesia, an upper jaw mucosa is incised from the vestibular side with its further skeletonization, detachment of the mucous membrane of the bottom and walls of the nasal cavity. Osteotomy of upper jaw is performed along Le Fort I lines, osteotomy of lateral walls of nasal cavity and lower portions of nasal septum along the whole length. Osteotomy is performed in a projection of a median palatine suture. Palatal distractor is inserted. Then, a rectangular window with sides length of 1.3-1.5 cm and height of 0.3-0.5 cm is created in the area of the Le Fort I osteotomy on the upper fragment of the upper jaw on the right in the area of its anterior part. Lead screw of a trans-sinus distractor is inserted through the created hole into the cavity of the maxillary sinus. Thereafter, the upper and lower external plates of the trans-sinus distractor are fixed with mini-screws to the upper jaw on both sides of the Le Fort I osteotomy. Drive of the trans-sinus distractor is directed into the oral cavity. Similar actions are performed on the counter-lateral side of the upper jaw with fixation of the second trans-sinus distractor. Further, test activation of the installed distraction devices is carried out and reverse twisting of the working parts of the installed distraction devices is carried out until contact of the upper jaw fragments along the lines of the performed osteotomies. Wound is washed and closed. Activation of the distractors is started on the seventh day after the surgical treatment.
EFFECT: method enables to achieve the planned jaw size in the treatment of upper jaw micrognathia in adolescence with the possibility of simultaneous upper jaw distraction in two mutually perpendicular planes: sagittal and transversal, without the need for multistage surgical treatment and the use of bulky external devices.
