FIELD: medicine.
SUBSTANCE: adipodermal flap is mobilized on the septal artery and the dermal-vascular pedicle. It is rotated by 180°. The distal flap segment is hemmed to the edge of the wound with a bend at maximum distance from the septal artery. The remaining part is placed in the formed bed. The donor and receptive wounds are sewn with the formation of a dermal cone at the base of the flap. Two months later, a skin incision is made along the edge of the flap, maintaining vascular connections to the skin of the recipient zone in the area of its distal segment. The dermal-vascular pedicle is crossed. The protruding part of the dermal cone is raised, forming a skin fold, its proximal leaf is crossed with the inclusion of the formed triangular flap into the adipodermal flap U-shaped flap, it is mobilized on the distal base, rotated by 180° and placed into the tissue defect.
EFFECT: method allows to replace the remotely located defects, increase the useful area of the flap, maintain adequate flap blood supply, minimize the donor flaw.
