SUBSTANCE: invention relates to medicine, namely to reconstructive surgery of the hand, and can be used for plastic surgery of the first toe with a non-free flap. A racket-shaped fat-skin flap is formed with a distal base and a length equal to the length of the metacarpal bone and the main phalanx of the first finger on a healthy hand. Without separating from its own fascia in the projection of the skin-fat flap and 2 cm away from its edges, a U-shaped flap is formed from its own fascia. On the palmar-radial surface of the distal radial bone, a periosteal flap is formed, which is associated with the bone graft and the radial artery. The radial artery is mobilized together with the intermuscular septum, all septal vessels along its entire length, as well as with skin-fatty, fascial and periosteal flaps as a single block. The resulting skin-fascial-periosteal-bone flap is moved to the skin-fascial-periosteal-vascular feeding pedicle in the recipient area.

EFFECT: method improves the efficiency of treatment, provides for the prevention of ischemic complications and bone graft resorption due to the combination of techniques of the claimed invention.
