SUBSTANCE: sanitation is performed by BTE, expanded radical operation with removal of all cells of the cellular tract of the temporal bone, skeletonization of the facial nerve channel, removal of the cochlea, hypothympanum sinuses and infralabyrinth cellular path to the periotic bone tip. The trepanation cavity is sanitised and the tympanic mouth of the auditory tube is closed with an autocartillage and fascial flap. At that, simultaneously with facial nerve channel skeletonization, intraoperative monitoring of the facial nerve is performed using needle electromyography by means of a dental drill prior to the emergence of threshold events with a current strength of 0.8 mA to 0.5 mA. After sanitation of the formed trepanation cavity, plastic expansion of the external auditory canal is performed, while the trepanation cavity is opened in the postoperative period.

EFFECT: decreased risk of relapse in patients with infra-labyrinth-apical cholesteatoma of the periotic bone with chronic sensorineural hearing loss of the fourth degree.
