SUBSTANCE: invention relates to medicine, namely to otorhinolaryngology. Perform an arcuate incision of the skin and soft tissues behind the ear. The skin of the walls of the external auditory canal is separated. Tympanotomy, canaloplasty, antromastoidotomy are performed with preservation of the posterior wall of the auditory canal, focusing on the horizontal semicircular canal. After that, a posterior tympanotomy is performed with access to the facial sinus of the tympanic cavity and a round window. Access to the hypotympanum and bulb of the jugular vein is drilled under the facial nerve canal. The tumor is removed, the walls of the internal carotid artery and the bulb of the jugular vein are examined, as well as all parts of the tympanic cavity and the mouth of the auditory tube. The eardrum is then put back in place. Fix it by tamponade of the external auditory canal. The ear wound is sutured in layers. Aseptic pressure bandage is applied.

EFFECT: method enables to provide good visualization of the anatomical structures of the middle ear during the operation without removal of the posterior wall of the auditory canal, simultaneously remove the paraganglioma, reduce the risk of intraoperative blood loss, preserve the middle ear sound conduction mechanism, and improve hearing.
