METHOD OF SUTURE OF APONEUROSIS AFTER LAPAROTOMY Russian patent published in 2018 - IPC A61B17/00

Abstract RU 2644846 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely, to surgery. Firming sutures are applied 5 cm along the incision line of the aponeurosis. When the reinforcing suture is applied, the first injection is made, retreating 1 cm from the line of the aponeurosis incision parallel to the last one with a puncture after 1 cm. Then, in the oblique direction towards the line of the incision and the first incision, 0.5 cm off from the edge of the incision, the indent is made on the opposite side of the aponeurosis in the oblique direction at the distance of 0.5 cm from the edge of the wound. Next injection is made in the same oblique direction as the previous pinch, but at the distance of 1 cm from the edge of the wound. Subsequent indent is done parallel to the edge of the wound at the acute angle to the previous course of the suture. Further, the incision is made in the oblique direction at the acute angle to the previous course of the suture at the distance of 0.5 cm from the line of the incision, and the indent on the opposite side of the cut in the same oblique direction at the distance of 0.5 cm from the edge of the wound. After that, knot is made. After the application of reinforcing sutures, the continuous seam is applied with stitches in staggered order with the use of the non-absorbable suture material. First, the interrupted suture is applied with distances from the edge of the wound on both sides of 1 cm, leaving one long suture. Next, the continuous seam with alternating small and large stitches is applied, the distance between the stitches is 0.5 cm, the distance from the edge of the wound in case of small stitches is 0.5 cm, and in case of large ones - 1 cm. After continuous sewing of the whole line of the incision, the sutures are tied at the end of the wound.

EFFECT: method reduces the risk of postoperative ventral hernias and eventions by means of increasing the scar strength.
