SUBSTANCE: invention relates to medicine, particularly to otorhinolaryngology. Perform an extended antromastoidotomy before visualization of the bone defect of the middle cranial fossa. Resect the cerebral hernia. Closure of the bone defect of the middle cranial fossa is performed, and an auditory and open sanitizing operation is performed. First, the bone marrow defect of the middle cranial fossa is closed, a plate-grid is placed between the cartilaginous plate and the fascial flap, which is wound behind the anterior margin of the bone defect, and the posterior part of it is sutured to the hole formed from the side of the cortical surface of the mastoid process. Then remove the cholesteatoma from the affected structures of the temporal bone and the plastic expansion of the external auditory canal, after which the tympanoplasty is performed and the antromastoidotomy cavity in the postoperative period remains open.

EFFECT: method allows to reduce the risk of recurrence of cerebral hernia and residual cholesteatoma, which is achieved due to the sequence of the above-mentioned operations.
