SUBSTANCE: invention relates to medicine, specifically to vascular surgery. Restoration of patency of the synthetic arteriovenous prosthesis for hemodialysis on the thigh of the patient is carried out by surgical intervention. In this case, a thrombectomy is performed with a Fogarty catheter before obtaining a satisfactory antegrade blood flow. In this case, during the surgical procedure, a 5–7 cm cut is made distal to the anastomosis zone. Then, the prosthesis is separated out over a distance of 4–5 cm, and a longitudinal prosthetomia 2.5–3 cm long is performed to perform thrombectomy. Within the operating wound, a segment of the large saphenous vein (LSV) is isolated, it is cut distally, subjected to hydraulic dilatation, dissected by the length of the prospective anastomosis with the prosthesis and anastomosing the LSV with an end-to-side type prosthesis.

EFFECT: method allows to increase the total time of functioning of the prosthesis, makes it possible to perform operations under local infiltration anesthesia, significantly reduce the total operation time, minimize intraoperative blood loss and reduces the risk of intra- and postoperative complications.
