FIELD: medicine.
SUBSTANCE: invention relates to medicine, specifically to surgical gynecology. Hold the front colporrhaphy with transverse access. Walls of the vagina are separated to the musculus obturatorius internus and the iliac spine on both sides. Sacrospinal ligaments are distinguished from two sides. Implant of titanium "silk" is used, the front central part of which is fixed to the wall of the vagina in the region of the border with the urinary bladder neck by two sutures at 11 and 13 o'clock. Similar manipulation is carried out with the lower edge of the mesh, which is fixed to the cervix uteri by two sutures at 5 and 7 o'clock. Using the Serapro instrument for through-out suturing the sacrospinal ligament, a bilateral through-out suturing of the sacrospinal ligament is performed in the region of its middle third followed by fixation to the lower angle of the implant extended up to 3–4 cm and by gradual tension to the physiological position of the anterior wall of the vagina. After pulling up to 3–4 cm, the front angles of the implant are sutured with the subsequent passage of the thread through the anterior transobturator way. Thread is tensioned until the angle of the mesh penetrates the musculus obturatorius internus until the physiological location of the titanium “silk” implant. Remaining defect of the anterior wall of the vagina is sutured.
EFFECT: method allows to minimize postoperative inflammatory complications, reduce the number of relapses of the disease, prevent the formation of dyspareunea, ensure the full sexual life of the patient.
