SUBSTANCE: invention refers to medicine, namely to traumatology and orthopedics, and can be used for treating periprosthetic infection in knee joint replacement. Method involves intraoperative sterilization of all components of the endoprosthesis. Intraoperative spacer is made for femoral and shin bone from bone cement with antibiotic in two-stage revision endoprosthesis replacement. Both spacers form cylindrical shape, length from 120 to 160 mm for femoral bone and from 70 mm to 120 mm for tibia, with diameter from 10 to 18 mm. Each spacer is reinforced with K-wire doubled to allow its two ends to have free length of 2 cm each. Free ends of K-wires are placed under components of re-installed components. Antibiotic added to the bone cement is pre-selected taking into account the verified pathogenic microflora: in the verified mixed flora, antibiotic Gentamycin 8–12 mg is added per 1 g of dry bone cement, with the verified gram-positive flora, antibiotic Vancomycin 25 mg per 1 g of dry bone cement is added, in verified resistant gram-positive flora, Linezolid 30 mg antibiotic per 1 g of dry bone cement is added to make spacers.

EFFECT: method provides higher clinical effectiveness in the periprosthetic infection in knee arthroplasty by adding an antibiotic, formed channels and sterilized endoprosthesis components into the spacer.
