FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to surgery, oncology and otorhinolaryngology. Subtotal laryngectomy is performed to form a tracheal-pharyngal shunt. Lower tracheostomy below the third tracheal ring is performed. After that, in conditions of mobilization of larynx from all sides with preservation of prelaryngeal muscles on healthy side of larynx vertical dissection of plate of thyroid cartilage is performed within attachment of pharyngeal constrictors. Suprahyoid pharyngotomy is performed. Subtotal laryngectomy is completed from top to bottom with removal of the entire hyoid bone and an epiglottis with preservation of one arytenoid cartilage and adjacent posterior one-third of the vocal and vestibular folds. After removing the thyroid cartilage plate residues on the healthy side, the tracheal-pharyngal shunt is formed on 14F catheter by stapling the rest of the mucosa of the posterior one-third of the vocal and vestibular folds with each other by means of interrupted sutures. Pharynx is closed with covering the pharyngeal suture and the tracheal-pharyngal shunt with the prelaryngeal muscles on the healthy side. Wound is layer-by-layer closed with active drainage left.

EFFECT: method enables the radical removal of the laryngeal tumor with the formation of the tracheal-pharyngal shunt in order to create favorable conditions for the voice rehabilitation of the patients suffering the locally advanced stages of laryngeal cancer.
