METHOD FOR SURGICAL TREATMENT OF LONG STRICTURES OF UPPER THIRD OF THE URETER AND URETERAL PELVIC SEGMENT Russian patent published in 2020 - IPC A61B17/00 

FIELD: medicine.

SUBSTANCE: invention refers to medicine, particularly to urology. Cicatrical-stenosed part of the ureter is incised and a buccal autograft is incised. Laparoscopic portion of the involved ureter, the pelvis-ureter segment and the pelvis portion with healthy tissue capture of 0.5 cm on each side are dissected. Internal drainage stent No.6 is placed in the ureter. Buccal graft flap is made of a buccal mucosa of 2.0 cm wide and delivered through a trocar working channel to a dissected pelvic ureteral area. Two threads are pre-extracorporeally applied to one of the narrow graft edges, and the mucosa is inserted into the proximal edge of the urinary tract incision: one suture is sutured to the medial edge of the incision with a continuous monocryl suture 4/0, and the second suture is sutured to the lateral wall of the dissected pelvic-ureteral region above the internal drainage stent by a continuous external monocryl 4/0 suture. Gland flap is cut out on the feeding pedicle with width corresponding to the length of the buccal transplant. It is brought to an anastomosis and placed without tension over the entire graft surface. Gland flap is fixed with several single sutures of monocryl 4/0 on the transplant edges with ureteral tissue capture and several single sutures of monocryl 4/0 directly to the transplant itself.

EFFECT: method enables reducing intraoperative injuries and reducing the number of postoperative complications and recurrences in the patients with recurrent strictures after previously unsuccessful ureter reconstruction.
