SUBSTANCE: invention relates to medicine, namely to oncology. Preparation is mobilized: spleen, tail and body of pancreas, unfractionated heparin 5000 IU is introduced intravenously. Vascular clamps are applied on the superior mesenteric and portal veins, the blood flow is stopped by the portal system. Circular resection of a portal vein confluence with a splenic vein mouth with tumor infiltration is performed. That is followed by an end-to-end anastomosis angioplasty between an end of the superior mesenteric and portal veins, wherein after suturing the posterior walls of the veins along the anterior wall of the portal vein in the longitudinal direction, a twisted suture is applied until narrowing the lumen of the portal vein similar to the diameter of the superior mesenteric vein. Further, the suture is continued cranially until the transition to the true diameter of the portal vein.

EFFECT: method enables performing the venous reconstruction of the mesenteric portal system, recovering the portal blood flow and reducing the risk of thrombosis of the reconstruction zone, development of intestinal and hepatic ischemia.
