FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to oncology. A single block with a gastro-pancreaticoduodenal complex with an inferior mesenteric vein is excised in conditions of complete blood flow arrest. A distal end of the superior mesenteric vein transversally crosses, the proximal end - the portal vein confluence intersecting obliquely at angle of 60-70° with resection of the right confluent wall and preservation of the splenic vein mouth. That is followed by reconstructing the superior mesenteric vein by prosthetics with a synthetic prosthesis; the diameter of the prosthesis is selected according to the diameter of the superior mesenteric vein within the resection area. An end-to-end distal end-to-end anastomosis is first applied between an upper mesenteric vein and a prosthesis, then an oblique anastomosis between the prosthesis and the portal vein confluence with preservation of the splenic vein mouth. Clamps with portal, splenic and superior mesenteric veins are used to remove and restore blood flow.

EFFECT: method enables reducing the risk of narrowing in the proximal anastomosis, venous thrombosis in the vascular reconstruction area, avoiding the use of autovein, reducing time and traumatism of the surgical intervention.
