FIELD: medicine.
SUBSTANCE: method for distal blocking of rods in intramedullary blocked osteosynthesis consists in the fact that after installing the distal target through the guide protector and the drill guide along the guide, the cortical bone is drilled with the spoke from the insertion side, getting into the lower distal blocking hole in the blocked rod, that is, a spoke is initially inserted into the distal blocking hole without dismantling the distal target. Then, the spoke is checked for entering the distal blocking hole with a guide spoke. If the blocked rod is cannulated, the guide spoke for introducing the blocked rod is carried in the distal direction along the rod channel, it meets the resistance of the spoke at the level of the lower distal blocking hole. If the blocked rod is uncannulated, then the check of the spoke getting is carried out using an electron-optical X-ray image converter. Then the opposite cortical plate is drilled, and only after that the spoke is replaced with a drill of the required diameter, by which the hole is drilled through the channel formed earlier by the spoke for the distal blocking screw, as well as after checking the drill getting in the distal blocking hole in the above-mentioned way. The guide-protector is left in the hole of the target. Through the guide-protector, a screw length meter is inserted into the hole drilled in the bone so that the hook of the meter reaches the plane of the “exit” of the hole. The screw meter is removed. Using a screwdriver, a blocking screw is screwed into the formed channel so that the screw head reaches the cortical layer of the bone. According to a similar scheme, there is a blockage in the upper distal blocking hole. Finally, an electron-optical X-ray image converter is used to perform radiography of the distal screws in two projections.
EFFECT: invention provides an increase in the strength of distal blocking of intramedullary rods, prevention of the occurrence of delayed consolidation and false joints, convenience in blocking non-cannulated blocked rods, reducing the number of intraoperative X-ray examinations, reducing the effect of X-ray irradiation on medical personnel and the patient, reducing the duration of the operation, the possibility of conducting without the use of electron-optical converter (EOC).
