FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to ophthalmology. A fragment of the internal boundary membrane (IBM) is formed around the macular rupture by separating and removing the membrane in the form of a rectangular frame, the guiding sides of which are parallel to the horizontal and vertical axes passing through the center of the macular rupture. At the same time, the membrane is separated and removed in 4 approaches, each time starting from the vertex of the frame, while maintaining the distance of the inner contour of the frame from the upper and lower edges of the macular rupture, as well as the right and left edges of the macular rupture of 0.8-1.0 mm, the distance between the inner and outer contour of the frame is 0.8 mm along the entire perimeter. Moreover, at the two corners of the frame, located diagonally, the guiding sides forming the corners are not completely closed, but a gap of 0.5 mm is left. In a particular case, each of the 4 vertices of the frame is defined as the intersection point of mentally drawn horizontal and vertical lines that are removed from the corresponding edge of the macular rupture: the right, left, upper and lower at a distance of 1.6-1.8 mm. In a particular case, first, having determined the lower vertex of the frame from the side of the temple, the vertical guide side of the frame is separated to the upper vertex of the frame from the side of the temple. Then, from the upper vertex of the frame from the nose side, the horizontal guide of the frame is separated until it joins the formed vertical guide. Then, from the upper vertex of the frame on the nose side, the vertical guide of the frame is separated to the lower vertex of the frame on the nose side. At the same time, separation begins while retreating from the formed horizontal guide frame of 0.5 mm, and then from the lower vertex of the frame from the side of the temple, the horizontal guide frame is separated until it joins with the formed vertical guide from the side of the nose. Moreover, separation begins while retreating 0.5 mm from the formed vertical guide frame from the side of the temple.
EFFECT: provides for the formation of a foveolar fragment of the internal boundary membrane in a low-traumatic manner, guarantees the closure of the edges of the macular rupture, as well as improves visual functions and preserves central vision in the early and long-term postoperative period.
