SUBSTANCE: invention relates to medicine, specifically to ophthalmology. For surgical treatment of macular ruptures, trocars 25G are inserted, access to the central fossa of the macula (foveolae) is formed by carrying out 3-port vitrectomy, contrasting and peeling of the internal border membrane (IBM), formation of the IBM flap and closure of the rupture with an inversed flap. Pre-operatively, microperimetry is performed prior to the operation to determine a new fixation point, in addition, at distance of 2 DD from macular rupture from the lateral side, a vertical notch is formed in the IBM, the IBM edge is raised, thereby forming a flap, and are partially separated from underlying layers of retina at distance 1 DD to center of rupture, after which flap is turned over and inner part is closed area of macular rupture so as to preserve new fixation point. IBM peeling in the papillomacular beam area is not performed.

EFFECT: method provides complete macular fracture closure, obtaining a high functional result and reducing operational and postoperative complications by eliminating IBM peeling in the papillomacular beam area and removing IBM.
