METHOD FOR ELIMINATING FUSION OF PALPEBRAL AND BULBAR CONJUNCTIVA Russian patent published in 2021 - IPC A61F9/07

Abstract RU 2759159 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to ophthalmology, maxillofacial and plastic surgery. After excision of scar tissue and dissection of adhesions, the true size and configuration of the defect zone is determined, according to which at least one template is made so as to cover the entire defect zone. If the template exceeds the size of the opening of the fenestrated clamp used for cutting out the grafts, it is divided into parts so that each of the parts does not exceed the size of the opening of the fenestration clamp, the required number of autografts is cut out according to the templates obtained in order to cover the entire zone of the palpebral and bulbar conjunctival defect. , while each autograft should not go beyond the opening of the used fenestration clamp and should correspond to the size and shape of the template according to which it was made. In this case, the cutting out of autografts according to templates begins in the area of the upper or lower lip, then a fenestrated clamp is sequentially applied on the cheek and on the opposite lip, leaving gaps with intact mucous membrane between the areas of autograft removal. Two types of grafts are cut out: split, which contains the epithelial layer to replace the defects of the bulbar conjunctiva, and the full-layer, containing the epithelial and submucosal layers, to replace the defects of the palpebral conjunctiva; split autografts are fixed to the edges of the main or residual defect and to the episclera of the eyeball over the entire area and in the area of the conjunctival fornix being formed at a distance of 12-15 mm from the limbus with U-shaped sutures, and full-layer autografts are fixed to the costal edge of the eyelid and the edges of the main or residual defect with interrupted sutures, in the area of the formed arches of the upper eyelid, retreating by 13-20 mm, and the lower eyelid, retreating by 9-10 mm from the costal edge, with U-shaped sutures, brought out to the surface of the skin and tied over the compression plates.

EFFECT: method allows replacing defects of bulbar and palpebral conjunctiva, including subtotal and total, ensures the formation or restoration of normal anatomy of the conjunctival cavity and arches while eliminating fusion of the palpebral and bulbar conjunctiva - simblefaron, ankyloblefaron.
