SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics, and can be used for plastic surgery of an old Achilles tendon rupture. The method includes replacement of the defect with a prosthesis made from a xenopericardium rolled into a roll, sewn into the split ends of the tendon, followed by restoring the sliding surface of the tendon by additionally wrapping the plasty area with a xenopericardial plate. A centrifugate from the patient's blood containing autologous plasma enriched with platelets is injected into the xenopericardium roll, after it is sutured into the tendon splits. At the final stage of the operation, platelet-rich autologous plasma is also injected under the xenopericardial plate, which wraps the plastic area.

EFFECT: method ensures complete restoration of continuity and sliding surface of the Achilles tendon and avoids undesirable complications and fully restores the function of the gastrocnemius muscle in the shortest possible time by introducing a centrifugate from the patient's blood and wrapping the plastic area with a xenopericardial plate.
