FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely, to coloproctology, and can be used for surgical treatment of chronic inflammation of the pilonidal sinus complicated by secondary fistulae of the gluteal and sacrococcygeal region in patients with flat gluteal configuration. The skin and subcutaneous fat are dissected using an electric knife in two semi-oval incisions at a step from the intergluteal cleft within the healthy tissue and 2 to 3 cm above and below the primary foramen of the sinus, capturing the secondary fistulous tracts without opening the lumen thereof. The pathology focus is excised within the healthy tissue in the form of a boat-shaped wound. The wound is closed from the bottom up by suturing the subcutaneous fat to 1/2 of the depth of the wound with nodal sutures made of a biodegradable suturing material, alternately, at a step of 1.5 to 1.8 cm. The ends of the threads of every other tied knot are then excised, then intracutaneous sutures are applied to the skin edges using the remaining threads of the knot. Next, the threads are tied together, wherein the skin edges of the wound are captured alternately, forming an intergluteal cleft in the form of consecutive semi-ovals divided by knotted intracutaneous sutures.
EFFECT: possibility of facilitating and shortening the operation and reducing the duration of postoperative recovery due to the minimal tissue tensioning, creation of the conditions for preventing circulatory disorder in the lowered wound edges, and ensured adequate outflow of wound contents.
