SUBSTANCE: invention relates to medicine, namely vascular surgery. The formation of an arteriovenous fistula after endovascular revascularization of the extremity is performed by a percutaneous puncture through one puncture hole under ultrasound control of the posterior tibial artery and its accompanying posterior tibial vein at the level of the ankle joint and inserting into each of them one conductor with a diameter of 0.014” to the level of the middle third of the thigh. After installation in the posterior tibial artery and the posterior tibial vein of each of the 0.014” conductors, their ends are brought inside the puncture needle, which is then pushed in the proximal direction under the skin from the site of the puncture hole of the posterior tibial artery and posterior tibial vein, while cutting medially adjacent intimately matted walls of the posterior tibial artery and posterior tibial vein from 2 cm to 3 cm with its edges.

EFFECT: method provides prevention of thrombosis of endovascular structures and an increase in their service life due to the formation of distal arteriovenous fistula after endovascular revascularization of the extremity, in addition, the method is low-traumatic, easily reproducible and does not require the use of additional endovascular tools.
