FIELD: medicine, endovascular surgery.

SUBSTANCE: percutaneous puncture is performed under X-ray control in the projection of the distal third of the stent using a puncture needle inserted between the cells of the stent. Then the angiograph projection is changed to accurately position the needle inside the stent. Recanalization is performed in the retrograde direction, a 4F multifunctional diagnostic catheter is inserted in the antegrade direction, and the guidewire is reoriented in the antegrade direction. The puncture needle is removed, manual hemostasis is performed. Then recanalization of the occluded area is performed according to the multifunctional diagnostic tool; a balloon catheter with Paclitaxel drug coating is inserted into the occluded area along the guidewire and predilation is performed for 5 minutes and a pressure of at least 8 atmospheres. After that, a self-expanding stent is positioned into the obtained lumen of the artery inside the stent. Post-dilatation is carried out with a balloon catheter, control angiography is performed.

EFFECT: method allows to increase the effectiveness of surgical treatment in case of repeated revascularizations of the femoral-popliteal segment by reducing the invasiveness of operations and reducing the frequency of postoperative complications.
