METHOD FOR HYBRID ARTERIALISATION OF DEEP VEINS OF FOOT IN CHRONIC ISCHEMIA, THREATENING LOSS OF EXTREMITY, AND VALVULOTOME FOR IMPLEMENTATION THEREOF Russian patent published in 2024 - IPC A61B17/11 A61B17/22

Abstract RU 2825708 C1

FIELD: medicine.

SUBSTANCE: group of inventions refers to medicine, particularly to cardiovascular and X-ray endovascular medicine. Popliteal artery or superficial femoral artery is approached, and a proximal anastomosis of the popliteal or superficial femoral artery with a greater saphenous or small saphenous vein is formed. Puncture femoral antegrade approach is performed; a valvulotome is put on the conductor and delivered to the shunt zone. Endovascular method involves destruction of venous valves by means of teeth on end of valvulotome, which is advanced in antegrade direction to ankle joint of saphenous vein. Valves of the medial marginal vein and the communicant with a deep venous plantar arch of the foot are destroyed by a balloon catheter. Lateral branches of saphenous vein and medial marginal vein are embolized by spirals. Method is carried out by means of a valvulotome consisting of a tube with two or more teeth at its end for destruction of venous valves and a lumen inside the tube for insertion of a guide conductor made with possibility of its antegrade passage.

EFFECT: group of inventions enables to create directed arterial blood flow through the venous system to the deep venous system, which promotes healing of trophic changes of the lower extremity and preservation of the extremity, and presence of at least 2 teeth at the end of the valvulotome allows destroying the valve apparatus of the target vein without damaging the wall of the shunt, wherein the start of blood flow to the foot through the saphenous vein by the endovascular method preserves the skin in the area of the foot intact.
