SUBSTANCE: invention relates to medicine, namely plastic surgery. With the help of a raspatory, the upper and lower temporal septa, lateral orbital thickenings are crossed by an acute method. Through a 1.8 cm long access along the transitional fold of the mucous membrane of the vestibule of the oral cavity, subperiosteal dissection of the anterior surfaces of the maxillary and zygomatic bones is performed. With the help of a raspatory, the supporting ligaments of the circular muscle of the eye, the ligaments of the nasolacrimal sulcus, the zygomatic ligaments at the place of attachment to the zygomatic bone are crossed by an acute method under visual control. The formed cavities in the area of dissection in the temporal and zygomatic areas are combined by dissecting the periosteum of the upper edges of the anterior two-thirds of the zygomatic arches using a raspatory using an acute method and forming a tunnel under the control of endovideoassistance. The mobilized tissues of the upper and middle zones of the face are tightened in the cranial direction and laterally at an angle of 35 degrees and their refixation is performed to the surface sheet of the deep temporal fascia through the tunnels formed by distant sutures with absorbable suture material at two points: the lower lateral part of the circular muscle of the mouth and the center of the molar sac. After tightening the tissues of the middle and upper third of the face, excess assessment and resection of the skin of the upper and lower eyelids are performed and a pressure bandage is applied for 3 days. The patient is discharged 3 days after the operation.

EFFECT: method makes it possible to mobilize, tighten and refix the soft tissues of the upper and middle zones of the face in a position that corresponds to the geometry of the face at the young age, and achieve a more stable result of the rejuvenating operation in the form of eliminating ptosis of the bodies and “tails” of the eyebrows due to the intersection of the upper and lower temporal septa, lateral orbital thickenings, eliminating the severity of nasolabial folds and palpepromalar grooves due to the intersection of the supporting ligaments of the circular muscle of the eye, ligaments of the palpepromalar groove, zygomatic ligaments, subperiosteal dissection of the anterior surfaces of the maxillary and zygomatic bones; eliminate signs of skeletonization of the lower edge of the orbit by applying a distant suture to the center of the molar sac; eliminate the omission of the corners of the mouth by applying a distant suture to the lower lateral part of the circular muscle of the eye; eliminate the likelihood of dry eye syndrome by assessing the volume of resection of the skin of the lower eyelid after tightening the tissues of the middle and upper areas of the face; reduce the time of wearing a circular pressure bandage by half.
