FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to maxillofacial surgery, plastic surgery, ophthalmology, and can be used in surgical correction of exophthalmos. Method for surgical correction of exophthalmos involves performing the following steps. Performing bilateral pinched blepharoplasty of upper eyelids, dissection of circular muscle of eye in region of upper eyelid on both sides, incision of orbital septum in region of upper eyelid on both sides, visualization and preservation of intact supratrochlear and supraorbital neurovascular bundles on both sides, visualization and preservation of intact superior oblique muscle of eye on both sides. Bilateral approach to orbital adipose tissue in the area of upper eyelid. Bilateral subciliary, paracanthal approaches. Dissection of circular muscle of eye in area of lower eyelid on both sides, dissection of orbital septum in area of lower eyelid on both sides. Bilateral access to orbital fat in the lower eyelid, removal of orbital fat, suturing with interrupted sutures from above and below of circular muscle of eye on both sides. Removal of true excess skin of lower eyelids, suturing of skin of lower and upper eyelids with intradermal sutures. After the orbital fat is removed, a bilateral subperiosteal dissection is performed within the inferior wall of the orbit. Infraorbital neurovascular bundles are visualized and kept intact on both sides. Inferior oblique muscle of the eye is visualized and kept intact on both sides. Double-sided reduction of the lower wall of the eye socket is performed with the help of a cutter. Mucous membrane of the maxillary sinus is kept intact on both sides. Providing bilateral approaches 1.5 cm long in the scalp of the temporal and frontal regions, performing the bilateral subperiosteal dissection in the frontal and parietal regions and the suprafascial dissection in the temporal regions, supratrochlear and supraorbital neurovascular bundles, a temporal branch of the facial nerve are visualized on both sides; the integrity of the supratrochlear and supraorbital neurovascular bundles and the temporal branch of the facial nerve are preserved on both sides. Bilateral approaches are performed within the vestibule of oral cavity 1.8 cm long. Bilateral subperiosteal dissection is performed along anterior surfaces of zygomatic bones and upper jaws. Infraorbital neurovascular bundle is visualized on both sides. Infraorbital neurovascular bundle is preserved on both sides. Flaps of the middle one-third of the face are moved in an upper-lateral direction on both sides. Flaps of the middle one-third of the face are fixed on both sides with the help of distant sutures to a superficial leaf of a deep temporal fascia. Flaps of the upper one-third of the face are moved upward on both sides; the flaps of the upper one-third of the face are fixed on both sides with removable intraosseous screws to the cranial vault in the frontal region. Temporo-parietal fascia leaves are closed with interrupted matching sutures on both sides, and scalp wound skin is closed with intradermal sutures. Operation is completed by closing the oral mucosa on both sides with continuous blanket sutures.

EFFECT: method enables to increase the support of soft tissues of the periorbital region, reduces the risks of postoperative recurrence of exophthalmos, diplopia and disturbed nasal breathing; solves problems of high risks of postoperative recurrence of exophthalmos, diplopia, disturbed nasal breathing, deficiency of support of periorbital tissues.
