METHOD FOR SURGICAL TREATMENT OF MACULAR HOLE WITH TARGETED PRESERVATION OF FOVEAL INNER LIMITING MEMBRANE IN THE AREA OF FIXATION POINT Russian patent published in 2023 - IPC A61F9/07

SUBSTANCE: three-port chromovitrectomy is performed with Triamcinolone acetonide dye, inner limiting membrane (ILM) is contrasted with the membrane blue dye 0.15%, intact ILM around the macular hole is preserved, the surgery is completed with air tamponade and ports removal. In the preoperative period determine the localization of ILM fixation point at microperimetry and its o’clock position. After that, two crescent-shaped flaps of the inner limiting membrane are formed, for doing this they are alternately captured and carried along an imaginary line enveloping the macular hole at a distance of 1.0 mm from its edge and the projection zone of ILM fixation point at a distance of 1.5 mm from the edge of the gap. At the same, one of the flaps is captured 2 mm from the inferotemporal arcade, and the other is captured 2 mm from the superotemporal arcade.

EFFECT: method makes it possible to eliminate retinal traction, reduce surgical injury, preserve fixation points due to intact foveal ILM.
