METHOD FOR SURGICAL TREATMENT OF IDIOPATHIC EPIRETINAL FIBROSIS WITH PARTIAL PRESERVATION OF INTERNAL LIMITING MEMBRANE Russian patent published in 2023 - IPC A61F9/07 

FIELD: medicine; ophthalmology.

SUBSTANCE: surgical treatment of idiopathic epiretinal membrane (iERM) is performed. Three-port chromovitrectomy is performed using Triamcinolone acetonide dye, contrasting the epiretinal membrane with membrane blue dye 0.15%, removing the epiretinal membrane, contrasting the internal limiting membrane (ILM), and removing it. Using endovitreal tweezers, the ILM is grasped at a distance of 3 mm from the inferotemporal arcade and 2 mm from the edge of the optic nerve head, a flap is formed, separating the ILM towards the center of the fovea, not reaching 1.5 mm from the foveal zone. Then the flap is drawn along the arc of an imaginary circle with a diameter of 5 mm, not reaching 2 mm to the optic nerve head. Next, the direction of ILM separation is changed towards the center of the fovea and the ILM is removed, not reaching 1.5 mm to the fovea zone, thus leaving the ILM fragment in the fovea and in the area of the papillomacular bundle intact.

EFFECT: method reduces the morbidity of surgical intervention, preserves the anatomical profile of the foveal zone, achieves maximum visual acuity in a short time, reduces the likelihood of central scotomas and improves the quality of vision of patients.
