METHOD FOR RETROPUBIC ADENOMECTOMY WITH CORRUGATED PLASTY OF PROSTATE CAPSULE Russian patent published in 2023 - IPC A61B17/00

FIELD: medicine; surgery.

SUBSTANCE: use technique of two purse-string corrugated suturing along the anterior surface of the prostate capsule. Isolate prostate gland and bladder neck, open the prostate capsule transversely, perform blunt and sharp enucleation of prostate adenoma nodes and hemostasis of the bed of benign prostatic hyperplasia removed nodes, closure of prostate capsule transverse section with continuous suture in two rows. At the same time, carry out corrugating plasty of the prostate capsule using two purse-string corrugated sutures along the front surface of the capsule parallel to the transverse section of prostate capsule. Make the first suture between the detrusor of bladder neck and transverse section of prostate capsule, retreating 0.7 – 1.0 cm from the lateral surface of prostate capsule and at least 0.5 cm from the detrusor of bladder neck. Make the second suture between the transverse section of prostate capsule and urethra 0.5 – 1.0 cm away from the lateral surface of prostate capsule, 0.7 – 1.0 cm distal to the transverse section of prostate capsule without involving the area of puboprostatic ligaments in the suture. The distance between starting points of purse-string sutures is 5 – 7 mm. After that, purse-string corrugated sutures are tied towards the center of the prostate capsule with a surgical knot and form corrugation of prostate capsule, insert drainage and suture postoperative wound by layers.

EFFECT: method allows to improve the urodynamic characteristics of the urine flow and reduce the number of postoperative complications.
