SUBSTANCE: invention refers to medicine, particularly to surgical urology. Surgical treatment of prostate adenoma is performed by extraperitoneal approach in patients with evident middle lobe, during which intraperitoneal approach is performed and prostate adenoma is removed. A resectoscope is inserted into the bladder, and the mucous membrane of the bladder is cut off from the middle lobe of the prostate under visual control by means of a loop electrode to form a visible border. Thereafter, a prostate capsule is dissected through the extraperitoneal approach, and the right and left lobes of prostate adenoma are harvested. Traction of the middle lobe is carried out and the previously marked border is visualized, enucleation of adenomatous nodes of the middle lobe is performed.

EFFECT: method makes it possible to increase the efficiency of the operation, to avoid the development of acute urination retention in the postoperative period, to reduce the number of postoperative complications due to visualization of the border and reducing the risk of damaging the ureter orifices, reduce the risk of bladder neck damage and, as a result, bladder neck sclerosis when hulling the middle lobe due to preliminary dissection of the bladder mucosa, reduce the risk of bladder sphincter injury and postoperative urinary incontinence.
