FIELD: medicine, cardiovascular surgery.
SUBSTANCE: operation is performed in two stages in one surgical intervention. At the first stage, with the patient lying on the back, the ascending aorta is isolated from the sternotomy access, the pericardium is opened longitudinally above the heart, the apex of the heart is brought up and to the left, the posterior pericardium is dissected above the diaphragm, and the thoracic aorta is isolated above the diaphragm. Then ascendo-descend bypass is carried out, while the prosthesis is carried out between the vena cava to the ascending aorta, the aortic arch is plugged and cut off between the left common carotid and subclavian arteries with implantation of the left subclavian artery into the left common carotid artery. At the second stage of the operation, with the patient in the right side position, a left-sided thoracotomy is performed in the 4th intercostal space, the aorta is isolated, plugged and cut off below the infection focus, the infection focus is removed along with the infected prosthesis or stent and surrounding tissues.
EFFECT: method allows to restore the main blood flow through the aorta bypassing the purulent focus with its removal, thereby significantly reducing the possibility of one of the most formidable complications — infection recurrence in the postoperative period.
